注意 : 凡訂購“ C - 2019「夢想．家」精裝畫集”者，請以高解像度jpeg檔 (學生相、證件相或生活照，必須近距離，頭部要大為佳) 電郵提交參賽者照片至 dc.icehk@gmail.com，檔案請以參賽者姓名及參賽編號命名 (e.g. Wong Tai Man 45678)
美元付款方式 (二 ) : 信用咭 - PAYPAL付款 (海外參賽者適用)
請登入ICEHK 網址 www.ice.org.hk 以信用咭付款，然後把信用咭 PAYPAL 之付款確認頁面掃描或拍照後，
連同本申請表格及參賽者照片(解像度為300dpi之jpeg檔 - 只適用於訂購畫集者)，
檔案請以參賽者姓名及參賽編號命名 (e.g. Wong Tai Man 45678)，並電郵至 dc.icehk@gmail.com 注意事項 :
(1) 所有費用收妥後，本會將於約1個月內以電郵回覆作確認，如逾期仍未收到確認書，請致電 (852)2646-2647查詢

The crossed cheque together with this application form shall be sent to "3/F, Toi Shan Centre, 128 Johnston Road, Wan Chai, Hong Kong" by mail.
Cheque Payee: ICEHK CONSULTANT LTD.
Please mark: 2019 ICEHK International Drawing Competition「Home Sweet Home」on the envelop.
Note:
- For those who have ordered item C (2019「Home Sweet Home」Artwork Collection Album), please send a softcopy of the participant’s photo (high resolution image captured by digital camera or captured by scanner at 600dpi with the participant number as file name) by email at dc.icehk@gmail.com. Hard copy of the participant's photo (4cm x 5cm) will also be accepted, however, the quality may not be as good as softcopy.
- Please mark down the name of the participant and the participant number at the back of the cheque and at the back of the photograph (if applicable).
Overseas Participants: Payment by Credit Card - PayPal in USD
Sign in ICEHK URL www.ice.org.hk for credit card payment by PayPal. On completion of the payment, please send the copy or photo of the payment confirmation page together with this application form to ICEHK by email at dc.icehk@gmail.com. Please use the participant number as the subject of the e-mail. For those who have ordered item C (2019「Home Sweet Home」Artwork Collection Album), please also send a softcopy of the participant’s photo (high resolution image captured by digital camera or captured by scanner at 600dpi with the participant number as file name).
Precautions:
- Upon receipt of the fees, we will send an e-mail to confirm payment received in one month time. If you have not received the payment confirmation e-mail in one and a half month time, please contact us at (852) 2646-2647.
- Please ensure the information that you provide to us are correct and we will not take any responsibility as the items are printed in accordance with the provided information. An administrative fee of HK$ 50.00 / USD 10.00 will be charged if a replacement certificate (item B) is required.
